Individualized Home Supports

This program offers in-home support based on individual needs, from 5 to 50 hours per week. Participants manage expenses through various means and work closely with staff to promote a healthy, integrated life. Funding sources include DDS, DMHAS, and private pay, with DDS waiver enrollment required.

The CRI Individualized Home Support (IHS) Program offers tailored in-home assistance for those living independently, with support tailored to each person's needs, ranging from 5 to 50 hours a week. Participants manage their living expenses through various means, including wages and subsidies, in homes they rent or own. Collaboratively, IHS staff, families, and agencies develop plans for a fully integrated life. Funding for support is available through DDS, DMHAS, or private pay, with DDS funding requiring Waiver Program enrollment.

 In-home staffing supports with individualized hours per Level of Need.

  • Assistance with personal funds/banking/paying bills/benefits management

  • 24 hour on call RN and residential support systems

  • Behavioral and activities of daily living teaching opportunities

  • Community based opportunities for recreation and socialization

  • Support in living a meaningful life as part of local community
