Community Companion Home

The Community Companion Home (CCH) Program is designed to provide a nurturing and supportive living environment for individuals with disabilities within the warmth of family homes across Connecticut. This program is ideally suited for individuals who thrive in a family setting and require assistance in several key areas of daily living and integration into the community.

Assistance Offered in the CCH Program

  • Medical Care: Individuals in a CCH receive comprehensive medical support tailored to their health needs, ensuring access to medical professionals and necessary treatments to maintain optimal health.

  • Daily Living Skills: The program focuses on enhancing the participant's ability to perform everyday tasks independently, from personal hygiene to cooking and cleaning, fostering self-reliance and confidence.

  • Money Management: Financial literacy and money management skills are imparted to help individuals handle their finances effectively, including budgeting, saving, and making informed spending decisions.

  • Community Activities and Recreation: Engaging in community events and recreational activities is encouraged to promote social interaction, community integration, and overall well-being. These activities are chosen based on personal interests and preferences, enhancing the individual's quality of life.

  • Ideal Candidates: The program is best suited for individuals who have previously lived at home with their families and are looking to maintain that sense of family life and comfort while receiving the necessary support.

Program Model and Support System

  • Family Home Environment: The CCH model emphasizes the importance of a family-like setting where individuals can experience the comfort and stability of home life while receiving personalized support.

  • Licensed Providers: Each individual is supported by a licensed provider who plays a crucial role in the care team. These providers are responsible for creating a welcoming home environment, participating in goal setting, and ensuring the individual's needs are met.

  • Collaborative Support: CRI staff closely collaborate with CCH providers and the individuals themselves to guarantee that the placement is not only suitable but also successful in achieving the desired outcomes for personal growth and integration.

Through the CCH Program, individuals with disabilities are given the opportunity to lead fulfilling lives in a family-oriented environment, supported by a network of professionals dedicated to their well-being and development.
