Community Living Arrangements

This program provides 24/7 care in a DSS-funded, DDS-licensed group home for 4-6 individuals with significant needs, offering occupational and physical therapy, dietary consults, daily activities, and RN support. It focuses on active treatment, skill-building, and community integration, catering to medically fragile individuals. Funded by DSS and monitored by Public Health and DDS, ICF emphasizes personalized care and community living.

The Intermediate Care Facility (ICF) provides services to individuals who have significant intellectual and physical needs. These ICF programs are also licensed CLA’s. These highly regulated programs are funded by the Department of Social Services (DSS), monitored by the Department of Public Health and DDS. The ICF programs follow both DDS regulation and Federal ICF regulations.

ICF homes are designed to provide active treatment in the least restrictive manner possible and to utilize an assessment process to determine individual needs and develop/monitor the implementation of programs. Individuals are supported in learning skills in household upkeep, leisure, participation in the community, choice making and habilitative therapeutic supports. Individuals are offered choices as part of their daily routine. They are supported to participate in religious and cultural traditions of their choosing, and are encouraged to foster relationships with friends and families. Highly trained support staff is available on all shifts to assist individuals with such things as lifts/transfers, positioning, bathing, dining, physical therapy needs and medical support. Each home has an activity schedule developed with individual choices and preferences in mind. Homes provide individualized program plans, leisure and community integration opportunities that are meaningful to the individuals. Several of CRI’s ICF programs provide 24-hour nursing supports and provide care to medically fragile individuals. Each program offers a safe, comfortable home environment and is located within a community neighborhood.

How this program works:

  •  24 hour supervision and support within a DSS funded and DDS licensed group home setting with 4 to 6 individuals

  • Occupational Therapy, Physical Therapy, Registered Dietician assessments and consults as needed for positioning, nutritional programs and guidelines

  • Daily activity schedules to provide active treatment with teaching opportunities, personal preferences, and community based recreation

  • 24 hour on call RN and residential support systems

  • Certain programs support very medically fragile individuals
