Intermediate Care Facility (ICF) 

ICF Group Homes

The Intermediate Care Facility (ICF) group homes operated by CRI provide services to individuals who have significant intellectual and physical needs.  These ICF programs are also licensed CLA’s. This highly regulated program is funded by the Department of Social Services (DSS) and monitored by the Department of Public Health and DDS.  These programs follow both DDS regulation and Federal ICF regulations.

ICF homes are designed to provide active treatment in the least restrictive environment possible and to utilize an assessment process to determine needs and develop/monitor the implementation of programs. Individuals are supported in learning skills in household upkeep, leisure, participation in the community, choice making and habilitative therapeutic supports. The staff also accommodates and assists individuals with significant physical needs such as lifts/transfers, positioning, physical therapy needs and medical support. Several of CRI’s ICF programs have 24-hour nursing supports and provide care to medically fragile individuals.   Individuals are offered choices as part of their daily routine, supported to participate in religious and cultural traditions, and foster relationships with friends and families.

Every home offers a safe, comfortable home environment with its own unique décor.  Each home has between 4 and 6 individuals residing in it.  Most have private bedrooms.  Highly trained support staff is available on all shifts to assist individuals in all activities of daily living.  Every home has an activity schedule developed with individual choices and preferences in mind.  Homes provide individualized program plans, leisure and community integration opportunities that are meaningful to the individuals.
