Community Residences, Inc.
operates Community Training Homes in the State Of
Connecticut for the Department Of Mental Retardation
North Central and North West Regions. Persons with
developmental disabilities live in the community in a
family atmosphere. All participants attend Day Programs
in their area daily.

Community Training Homes can support
people with all levels of disabilities successfully
within the community. Individuals have been diagnosed
with a variety of disabilities including physical,
intellectual and/or psychiatric.
People who are successful in a CTH
are those who have thrived at home and are comfortable
with the daily routines of a family. Individuals
currently living in CTH’s throughout the State range in
age from eighteen through their eighties and even into
their nineties. Placement in a
Community Training home allows for continued growth and
independence in the least restrictive setting. One can
become independent while enjoying the nurturing
atmosphere of a family.
The placement is supported by
benefits awarded to the individual through the
Department Of Social Services and Social Security and
supplemented by the Department Of Mental Retardation.
Stipend rates paid to the provider are determined by the
supervision needs of the individual and the individuals
Treatment Team through the Department Of Mental
Retardation.
The individual
pays a monthly room and board fee established by the
Department of Social Services directly to the provider.
Routine transportation to medical appointments and
recreational activities is also included. The individual
is responsible for clothing and all recreational
activity fees. Medical expenses are covered by the
individuals insurance company.
DSS provides an allowance of $119.60
per month to provide for the individuals discretionary
needs. Provider rates have
been set by DMR. Providers can earn from $1017.51 to
$1560.68 depending upon the individuals abilities and
skills who is living in their home.
Community Residences, Inc. provides
Case Management, on-going training and placement
supervision ensuring that the needs of the individual as
well as the provider are met. Nurses are assigned to the
home to help ensure that the medical needs of the
individual are met.
Individual Community Training Home Providers are
responsible for providing daily training in activities
of daily living as identified by the Interdisciplinary
Team (IDT) during the Treatment Team meetings.
Individual areas needing improvement are identified and
goals developed to increase skill acquisition.
Referrals are made to Community
Residences, Inc. by DMR. Referral information is
reviewed by the Community Residences, Inc. CTH Program
Supervisor to determine the needs of the individual. A
review of the Programs openings is conducted to
determine if one of the existing providers can meet the
needs of the individual. If a match can be made, the
IDT’s Case Manager is notified and the placement process
begun.
Community Residences, Inc. will
arrange a meeting between the individual and the
provider at the provider’s home. Guardians are
encouraged to participate in this introductory meeting
to ensure that the placement will meet the needs of the
individual. After the initial introduction, a number of
visits will be scheduled between the provider and the
individual allowing for them to become acquainted and
understand the expectations prior to moving. The number
and duration of each pre-placement visit is determined
by the Interdisciplinary Team. Once the provider and
consumer are comfortable, the placement date is
determined.
